try making the entire name lower caps
Folder > \Everquest\SKins\bellsix
imagesXML > \Everquest\Skins\bellsix\bellsix.xml
Skins Tag > <skin>bellsix</skin>
also you might want to check the name of the modded background you want to use (if you have changed the XML that is related to the images file the skin will not load)
*IE the background MUST be called main-bg.png
