Small update for the recent patch.
If you already use this UI, just patch with the latest modified files I did.

That should be fixed I think. Let me know if it is still a problem with the default_old file.

Not sure what you are trying to do. Check your UIerrors.txt. That would give you a clue at the end as to what it is erroring on.
You should not be touching the "default" folder at all. You also should be installing a Fresh copy, not overwriting what is there already. When you extract the zip to the UIfiles directory, you should only see 2 folders then.
\uifiles\default
\uifiles\default_old
Open up default_old, and insure there are a whole bunch of files there. If you see another folder instead, then you extracted it wrong. In that case, just cut and paste the folder back a notch.
I have extensively checked this folder for errors and crashes, and at this point, chances are, it is extracted wrong to do that.

I've tried copying/renaming 'default' and piecing in certain parts of 'default old' like spell book and tried to /load but it won't take the update...it just shows 'default'.
Any ideas?

Removed the graphics as in, we can't see these anymore either?? (I'm tired, maybe I'm not reading this correctly. lol)

I updated the file today just because they had a few out-of-place characters in the guildmanagement window that could pose a crash, so I fixed it and emailed sony about the problem.. maybe they can fix the normal default one too /shrug.

If you want the small icon buff windows, just take the shortdurationbuffwnd and buffwindow xml's from the default and paste them into the default_old. It wont crash it.

Re: buff icons
Buff Icons are like they always were for default_old for me. Don't see any bug there
