IF you have a custom EQUI.xml file, open using a text editor (Word, WordPerfect, NotePad, etc.) and using the Search or Find function:

Find:

<Include>EQUI_FindLocationWnd.xml</Include>

Immediately after that line, ADD:

<Include>EQUI_MeleeBuffWindow.xml</Include>
