Solve the biggest one first, do you actually have a customized EQUI_BugReportWindow.xml? If it isn't actually customized, just delete it from your custom folder, the changes required to make it current are a pita.

allright i got my EQUI.xml file open on notepad and i got the patch fix open in a window....

Okay, let's start from the top.
First question ... did you copy the files from the default folder into your custom folder? If so, heh, don't. Delete everything in your custom folder and re-unzip the mod you want to use into it. The *only* thing that you need in your custom folder is the files that have actually been changed.
(And yes folks, we are walking through this step by step, it's the easiest way for both of us. *grin*)

To solve the one that had the most errors first, is there a file called EQUI_BugReportWindow.xml? If so, and if it is *not* customized, delete it.

k, open it up using your NotePad
down at the bottom of the <Include> lines, add:
<Include>EQUI_ShortDurationBuffWindow.xml</Include>
<Include>EQUI_StoryWnd.xml</Include>
Save and close

Open the EQUI_PetInfoWnd.xml file (using Notepad)
Find:
<TooltipReference>Pet Sit Down/Pet Stand Up</TooltipReference>
Then DELETE the
/Pet Stand Up
portion of it.
Scroll to the bottom of that bit of code until you see:

heh, fun, hey? *grin*
Okay, looking at your error log again, check and see if you have EQUI_OptionsWindow.xml file in your custom folder. If you do, delete it. I *highly* doubt that it is customized.
Also, if you have EQUI_LootWindow.xml in your custom folder, is it actually a custom loot window? If not, you know what I'm gonna say ... delete it.
Ditto that for the EQUI_SelectorWindow.xml.
