Try the attached. I compared the .ini one that I know works, and made some adjustments. I think the main thing was that markers 4-6 were not defined.
If it doesn't work, then it is something beyond my skills.

This is the Ring I used in Deltax Forever, just rip it out of there.

Intermittent spikes
The odd thing is that sometimes it will still give me the smoky target ring and not the spikes. Any ideas what might cause that? I can click on my group and 4 of the 5 show spike ring, and consistently one person will show the default.

not 100% sure but from what i have seen over the years it seems to be a directx issue or memory leak that lingers and becomes progressive the longer eq has been running and most common after reloading the UI a couple times.

A problem I encountered with a custom target ring recently, some mobs had a smoky ring instead of the custom ring. The issue was sony added another con level. It's either between dk blue and lt blue or lt blue and even. I forget exactly. So when targeting a mob with the new con the ring defaulted to smoky. The new con had to be separately defined. Hope this helps.

Pintoy was exactly correct. There was a level of con that was not defined in the .ini that I included. Here is the corrected files.

When I try to copy these 3 files it won't allow me to copy the TGA files, I get an unspecified error
I'm hoping the ini file is the only real change I need and it will work with the other TGA files I have.

Yes, the only thing I changed was the .ini file. Weird that it broke the tga files. I do use Power Archiver here at work... which a bit ole piece of junk.
Just in case, I rezipped and attached.
